DAY-TO-DAY CHRONIC BENEFITS

If you have a condition that requires ongoing medication (minimum of three
(3) months), please ensure that you complete a chronic form and submit it
to our office. The purpose of Chronic Registration is to ensure that members
receive sustainable funding for cost-effective treatment. It also helps in ensuring that the chronic condition is well monitored and the member adheres to treatment hence preventing the occurrence of complications.
